TO IMPROVE SAFETY FOR THE SENIOR IN YOUR LIFE, AN EXPERIENCED HANDYMAN WOULD BE A BETTER OPTION

You might be the classic, modern do-it-yourselfer. That’s a wonderful thing. Being able to do any number of home improvement projects on your own is not just a wonderful way to save money, but also feel satisfied. It’s a sense of pride for many men and women to be able to fix something in the house, repair a leaky faucet, or even open up a wall in the house to create a more open atmosphere.

What kind of mistakes could an inexperienced do-it-yourselfer make?

Let’s talk about grab bars. These are not the same as towel bars because they are designed to hold up the weight of a full grown, healthy adult. A towel bar is most likely simply secured to the wallboard or tile. If you held onto that towel bar, put your full weight on it, what’s going to happen?
It’ll probably rip right out of the wall, damaging the tile or vinyl surface as it does. What would’ve happened if that aging senior tried to grab onto the towel bar when they were slipping?
It could very well lead to even more significant injuries as a result. Now, let’s say you understand the importance of a grab bar to keep this aging senior safe. Let’s say, also, that you installed it just as you would have a regular towel bar.
You may have used a device to try and find the wall studs so you could anchor them properly, but because of the tile and the type of wallboard, you got false readings. You didn’t fully understand those false readings, so you anchored the grab bar where you thought a wall stud was.
But it wasn’t there. Suddenly, a false sense of security was provided to that aging senior in your life and they grabbed onto it. It ripped out of the wall and they fell, suffering serious injuries.
What went wrong? There could be any number of reasons why that wall stud finder produced a false result. If you don’t have experience, like somebody who has been doing this for decades, the result could be devastating. That’s why it may be best to depend on an experienced handyman for these home modifications when the safety of seniors is at stake.

How did Perfect Solutions for Seniors come to be? It all started with Tessy and a need she saw in the Senior community.

“I grew up in Peru and moved to the U.S. when I was sixteen. I was very young and green, scared,  but I knew the opportunities that the U.S. would bring. Myself and my 5 other brothers and sisters, worked hard to become citizens and made our lives in this country.
During this time, my mom wanted to stay in Peru though we all wanted her here in the U.S. with us. When she no longer could care independently for herself, my family and I didn’t know what to do. We were all here, with our families and responsibilities. It was a tough emotional time for me, not knowing what I could do for her, being thousands of miles away.
We thankfully found a live-in to be with my mom in Peru, and it was such a blessing and relief knowing that my mother could go on living in a home that my family bought for her.
I realized that there are other families that are going through the same thing, and knowing the frustration and turmoil my family and I went through looking for someone to help my mother in a different country, I looked into starting a homecare company. When Perfect Solutions for Seniors was founded in 2005, I was a one-woman show. I was the owner, receptionist, administrator, marketer and even the caregiver!
 I am no stranger to hard work and starting from the bottom! It was difficult at the beginning, but I believed in providing great care for Seniors. For 11 years Perfect Solution for Seniors was a homemakers and companion company until I spoke to my daughters and we decided to become a home health agency. With Francesca’s intelligence and Cassandra’s dedication we obtained our license, in 2018.

Communication is the foundation of our relationships. Communication is not only words but also gestures, facial expressions, body positioning, and touch. Having a speech-language pathologist on our team always educating the team is vital for increased quality of life for our clients.

What is VA Aid and Attendance?

If you are a veteran overwhelmed with the high cost of long-term elder care, such as paying for assisted living facilities, home care aids, adult daycare, or skilled nursing, the Veterans Aid and Attendance benefit could be the solution to help offset these rising care costs. A wartime veteran or their surviving spouse with limited income may be eligible to receive a non-service connected pension (this means that the need for care does not have to result from one’s military service).
There are three levels of VA Pensions:
  • Basic Pension / Improved Income – for healthy veterans over the age of 65 with low incomes
  • Aid & Attendance (A&A) – for veterans over the age of 65 that require assistance with their activities of daily living
  • Housebound – for veterans with a disability rating of 100% that prevents them from leaving their home, but does not have to be related to their military service.
The Aid & Attendance and Housebound pensions provide additional monthly income over and above the Basic Monthly pension. To be eligible for either of these pensions, one must also meet the requirements for the Basic Pension

MYASTHENIA GRAVIS

Myasthenia Gravis (MG) first appeared in medical reports in 1672, but was not named till the 1880’s. Myasthenia Gravis literally means “grave muscle weakness”, and is an autoimmune disorder affecting the neuromuscular junction between the nerve and muscle.






Symptoms include :

Drooping of one or both eyelids ( ocular MG)
Double or blurred vision
Weakness in arms, hands, fingers, neck, face or legs
Difficulty chewing, smiling, swallowing or talking
Excessive fatigue in exercised muscle groups
Shortness of breath

MG most commonly impacts young adult women ( under 40) and older men ( over 60), but can also occur in children. Approximately 15-20% of people with MG will experience a myasthenia crisis, where individuals respiratory muscles will weaken and a ventilator is required. This crisis may be triggered by medication, stress, or infection. It is vital that the individual is followed closely by a Dr.

Due to MG being a lower motor neuron disease, exercise or any activity will not help, in fact it is contra-indicated. Though at this moment  there is no cure, there are ways to control MG.

1. Thymectomy- is an operation to remove the thymus glands and can assist in reducing symptoms

2. Antiocholinesterase Medications – these medications slow the breakdown of acetylcholine at the     neuromuscular junction

3. Immunosuppressive meds- these meds MUST be carefully monitored by a physician

COMMUNICATION TIPS FOR PEOPLE WITH DEMENTIA

Here are some useful tips to effectively communicate with a person with dementia. Communication is not a one size fits all, so please tailor your approach to each individual, and remember sometimes its not WHAT you say but HOW you say it.

Begin by Setting a Positive Mood- Your attitude and body language communicate stronger than words.

Be sure to get the persons attention-Limit distractions and noise. Address client by name, identify yourself by name and use non verbal cues and touch to help client stay focused. If client is seated, get down to her level and maintain eye contact.

State your Message clearly. Speak slowly, and distinctly and in a reassuring tone while using simple words and sentences. Ask simple, specific, answerable questions. Only ask one question at a time. Questions with yes or no answers work best. Give instructions that are easy to understand. Break down tasks into simple 2-3 simple steps

Adjust your expectations. Respond with affection and reassurance. Do not use “remember”. Do not test their memory or tell them of their deficits.

When the going gets tough, distract and redirect. By redirecting an individual with dementia, you may be able to avoid or delay outbursts or inappropriate behaviors.

Use validation.  Use humor whenever possible.

INCLUSIVE ACTIVITIES FOR SENIORS DURING THANKSGIVING

Holiday Activities & Home Care



Imagine your house full of people you love, endearing conversations being had between family members, loved ones yelling at the T.V. for sports games, children chasing each other from room to room developing life long friendships with their cousins. Sounds amazing right?

Now imagine you are a Senior, hard of hearing, trying to remember who these people are, attempting to remember “Why am I here again?”, “What are we celebrating?”. A family member asks you a question, and a well meaning daughter or spouse answers for you, robbing you of the opportunity for a social connection. The emotion is much different, right?

Holidays are a joyous occasion but can also be overwhelming for someone with Dementia or even someone who is hard of hearing. Here are some suggestions to help create a welcoming atmosphere:

1. Have a photo album labeled with names, and reminisce with them multiple times a day with different family members who can offer different perspectives of the same story

2. Incorporate your Senior in activities such as cooking. Have them sit at a table, and mix apples for the apple pie, or fold the napkin linens for the table.

3. Have a room designated as a “quiet room”, where one-on-one conversations can be had without environmental distractions. This room is not meant to isolate a Senior, but to increase the social connection that is vital for their quality of life.

4. Have a schedule ready to show them the events of the day so they are aware of what is next (this also helps you in organizing your day!)

We often see loved ones do everything for their Senior, thinking they are doing best for them, but often times we are handicapping them. Taking away what they still can do for themselves, and creating dependency. Seniors, even those with Dementia, can still contribute to daily activities, but just with some modified adjustments.

GENERAL TIPS TO IMPROVE MEMORY



1. Be alert and aware

You cannot remember if you do not attend to the item you want to recall. When you pay attention you will become aware of things you ordinarily will miss.

2. Visualization/ Association

Memory is based on associating new information to something you already know. It helps to link what you want to remember to a strong visual image. Example, people often remember the shape of Michigan because they associate it with a mitten. Or next time you want to remember someone’s name, associate it with someone you already know or a famous person.

3. Repetition

In order to place something in your memory bank, you must repeat it at least 3 times. Verbal repetition linked with a visual image can increase your ability to recall

4. Organize new information

Catagorize new information to help you remember or use acronyms.

5. Routine

Do things in the same order/time every day to help you recall

6. Write it Down

Use a planner

PARKINSON’S AWARENESS MONTH

April is Parkinson’s awareness month. In simplest terms, Parkinson’s Disease (PD) is a central nervous system disorder than can affect movement, often involving tremors. These tremors can be exhibit themselves in outer extremities such as legs
and arms, or internally such as one’s larynx impacting their ability to swallow.



Though many are aware of the term Parkinson’s, few know early signs of Parkinson’s.

The Parkinson’s Foundation lists 10 early signs of “PD” Parkinson’s Dementia.
1. Tremors
2. Small Handwriting
3. Loss of smell
4. Trouble sleeping
5. Trouble moving or walking
6. Constipation
7. A soft of low voice
8. Masked face
9. Dizziness or fainting
10. Stooping or Hunching Over

A symptom of Parkinson’s that can be easily overlooked is dysphagia, which is a swallow impairment. Tremors present in the larynx impact the safety of the swallow mechanism increasing the individual’s risk of aspiration and aspiration pneumonia. Aspiration is when a foreign particle, such as a liquid or food, enters past the vocal folds of the trachea and into the powerhouses of our body: our lungs. Speech Language Pathologists (SLP) are the healthcare practitioners that will work within the interdisciplinary team to provide diagnosis and treatment for dysphagia.
There are 3 types of dysphagia:

1. Oral Dysphagia: swallow impairment that occurs inside of the oral cavity. Impairments generally are impaired mastication, or chewing, of food, and/or decreased propulsion of the food or liquid bolus resulting in oral residue.

2. Pharyngeal Dysphagia: swallow impairment that occurs in the pharynx which can result poor transit of the bolus to the esophagus or decreased coordination of pharyngeal elements resulting in aspiration.

3. Esophageal Dysphagia: impairment in the esophagus where bolus does not transmit to stomach area. A referral to a GI specialist is needed.

An individual can experience dysphagia in multiple phases of the swallow. If you are experiencing any of the above symptoms in swallowing or early signs of Parkinson’s, consult with your Doctor who can make the appropriate referrals. As Parkinson’s progresses, more assistance may be required to support you in your activities of daily living such as meal prep, housekeeping, and medication reminders. AMYOTROPHIC LATERAL SCLEROSIS (ALS)

ALS, amyotrophic lateral sclerosis, also known as Lou Gehrig’s disease (baseball’s “Iron Horse”), is a progressive neurogenerative disease impacting the nerve cells of the brain and spinal cord. When these motor neurons die, the brain’s ability to initiate and control those muscles are lost. Two types of ALS are:

Sporadic- most common form
Familial-the disease is inherited


The Mayo Clinic cites these early signs of ALS:

1. Difficulty walking or doing your normal daily activities
2. Tripping and falling
3. Weakness in your leg, feet or ankles
4. Hand weakness or clumsiness
5. Slurred speech or trouble swallowing
6. Muscle cramps and twitching in your arms, shoulders and tongue
7. Difficulty holding your head up or keeping good posture

NUTRITION AND AGING

As we age, our needs change. Physically, emotionally, and mentally we are needing different things to satiate us. What most often gets overlooked is that nutritionally our needs change as well.



As we age, our resting metabolic rate declines, which can lead to weight gain and increase your risk for chronic illnesses. At the same time, we are losing lean muscle mass. So what can you do to decrease this?

DANGEROUS UTIS CAN FOLLOW HOSPITAL PATIENTS HOME

For the sick or elderly, a urinary tract infection (UTI) can prove deadly. With many vulnerable patients developing UTIs post-discharge, a new study suggests that better monitoring is needed after leaving the hospital.
Researchers at Oregon State University explored more than 3,000 at-risk patients. The study revealed that the risk of infection tripled once patients went home.
UTIs are the most common type of health care-related infection, according to the National Healthcare Safety Network. While the infection is usually harmless for the young, it can have deadly consequences for the elderly.
“If left untreated, UTIs can progress to more serious disease such as sepsis,” said study author Jessina McGregor, associate professor at OSU’s College of Pharmacy.
Catheters — thin tubes used to drain urine from the bladder in bedbound patients — cause most hospital-acquired UTIs, the report noted.
Though severe UTIs can cause symptoms such as vomiting, fever, shaking and chills, a mild infection can be difficult to detect at first.
“In elderly patients, diagnosis of urinary tract infections can be challenging since advanced age also results in some of the symptoms often attributable to UTI, such as increased urination frequency or urgency,” McGregor said
A constant urge to urinate and pain or burning while urinating are hallmarks of a UTI. Once discovered, the infection can be treated with antibiotics. But the issue is twofold: If a doctor misdiagnoses symptoms of aging as an infection, McGregor is unsure if the cost of treatment outweighs the precautionary benefits.
“We are worried about elderly patients being treated for UTI when in fact their symptoms are not due to an infection,” McGregor said. “Unnecessary use of antibiotics puts the patients at risk for drug-associated side effects without offering any potential benefits.”
The study showed that about 11 patients per 1,000 developed a UTI in the hospital. About 30 per 1,000 developed a UTI within a month of going home. As patients are experiencing shorter hospital stays than ever, improving the transition from hospital to home is essential to solving this problem, the researchers said.
“During transitions of care, health care providers need to have open lines of communication to ensure they have a complete picture of a patient’s medical history when providing care,” McGregor said.
Dr. Thomas Finucane, a geriatrician at Johns Hopkins Bayview Medical Center in Baltimore, explained why providing antibiotics for unconfirmed UTIs isn’t favorable.
“We used to think the urinary tract was sterile; this was the basis for many decisions about giving antibiotics,” said Finucane, who wasn’t part of the study. “But now we know that the urinary tract has a microbiome and is, in fact, never sterile.”
The lack of sterility in the urinary tract could mean that some symptoms of a UTI can be present in healthy individuals. This idea implies that the UTI diagnosis is overused and patients don’t always need antibiotics.
“In most cases, if a young or middle-aged woman develops painful urination, it’s safe to take pain meds and observe carefully,” Finucane said. “But most of the time it gets better by itself.”
The study can’t prove that the infections were picked up in the hospital. But the link is strong enough to support further research, McGregor said.
